AI Scribe

Claude-powered SOAP notes + ICD-10/CPT coding generated from the visit conversation — cuts provider documentation time by 60–80%.

AI Scribe is an in-room AI agent that listens (with patient consent) during the visit and generates a complete SOAP note, ICD-10/CPT codes, and a claim-ready summary — eliminating after-hours charting and putting providers back in front of patients.

What it does

Provider burnout is driven primarily by documentation. AI Scribe reduces note-taking time by 60–80% by capturing the visit, structuring it into a compliant SOAP format, and pre-coding the diagnoses and procedures. Providers review and sign off in 60 seconds instead of typing for 15 minutes.

How it works

Patient consent is captured digitally (compliant + auditable). The provider clicks “start visit” and the agent listens via the room mic or a phone app.

Step 2 — Structure the note

Claude-powered model converts the conversation into Subjective / Objective / Assessment / Plan, with proper medical terminology and the right level of detail per visit type.

Step 3 — Code & submit

ICD-10 and CPT codes are suggested with confidence scores. Provider reviews, edits if needed, signs, and the note flows into the EHR + claim queue.

What you’ll see in 90 days

  • 60–80% reduction in provider documentation time
  • 2 hours/night recovered per provider — burnout dropping fast
  • Coding accuracy +10–20% vs. provider self-coding
  • 10–15% more visits/day possible because providers aren’t buried in notes

Setup & onboarding

Live in 10–14 days.

  • Days 1–3. Consent flow, EHR mapping, specialty-specific note templates.
  • Days 4–8. Pilot with 1–2 providers. Tune note style, terminology, length.
  • Days 9–14. Roll out clinic-wide.

Common questions

The provider can opt to chart manually for that visit. The system never records without consent.

Does it support every specialty?

Yes — note templates exist for primary care, derm, plastic surgery, pediatrics, urgent care, ortho, gastro, and more. Custom specialties take 1–2 weeks to tune.

HIPAA + audio data?

Yes. BAA included. Audio is processed and discarded by default; only the note + transcript persist (configurable).

Pricing

$499/month per provider. Includes the model, EHR sync, and unlimited visits. Cancel anytime.

Who this is not for

This agent isn’t a fit if:

  • Telehealth practices with poor audio quality — transcription accuracy drops.
  • Specialties not yet supported (rare exotic specialties — we'll quote a custom build).
  • Clinics whose providers don't review notes before signing — Scribe assists, never charts autonomously.

Troubleshooting

SOAP notes feel too generic

Add 10+ specialty-specific note samples to tune the template.

ICD codes wrong

Provider corrects in review; the system learns from the corrections (typically 60 days to convergence).

Audio not capturing

Mic permission or hardware. Check device settings and browser permissions.

Notes not saving to the EHR

API auth needs a refresh — the most common failure mode.

Pair this with

These agents are designed to work alongside this one — clinics typically activate them together:

  • Patient CRM — A purpose-built healthcare CRM where every patient, call, text, appointment, and payment lives in one searchable record — auto-tagged and scored.
  • No-Show Recovery — Automated win-back sequences that fire the moment an appointment is missed — recovers 30–50% of revenue that would otherwise be lost.
  • AI Workflow Automation — "When X happens, do Y." No-code automations that connect every tool — eliminating the copy-paste tax your staff pays daily.
